In order to become a Diego Kush Caregivers patient member, you must first pre-verify online and complete the new patient registration form. A Diego Kush Caregivers membership is free for all certified San Diego medical cannabis patients for which you are entitled to receive home delivery of medical marijuana legally and for personal use only!
- PRE-VERIFY online by completing the New Patient Registration Form.
- DOWNLOAD, PRINT OUT & SIGN the Diego Kush Caregivers, Inc. Membership Agreement.
- STAND BY to receive a phone call from a Diego Kush Caregivers authorized representative who will notify you that your original doctor's recommendation has been approved and verified with your physician.

**PLEASE NOTE** If you are unable to SCAN the required copies of your original doctor's recommendation or your California ID(s) to complete the online registration process, then you can simply take a picture of your recommendation and California ID and upload the photo.
